  MDS 3.0: Section O, Therapies
 
CMS posted an update to MDS 3.0 RAI Manual, Section O in June 2010.  This section provides information on what consitutues skilled therapy for calculating minutes to be coded in O0400A,B,C (Speech, physical and occupational therapy). Only skilled therapy services provided within the look-back window of the ARD are coded and must meet the conditions outlined in the Medicare Benefit Policy Manual, Chapters 8 and 15.
 
One of the notable newer items in this section deals with the clarification of
coding minutes of therapy. On page O-18 the manual elaborates on non-skilled services.  The following paragraph will require therapy professionals to review the current minute allocation practices for their department:
  • When a resident refuses to participate in therapy, it is important for care planning purposes to identify why the resident is refusing therapy. However, the time spent investigating the refusal or trying to persuade the resident to participate in treatment is not a skilled service and shall not be included in the therapy minutes.
 
Harmony recommends that therapy team members review the guidence in this section and develop a reference for all therapy staff members to utilize in determining when services provided rise to the level of skilled care.
