New CMS MDS 3.0 Web Page
 
May 2010: CMS has a couple of new links on their MDS 3.0 for Nursing Homes page. CMS has created a Training Materials page in addition to the Technical Information page and Training Conference Information page.  CMS is proposing to conduct another Train-the-Trainer conference in August. Log on for more details.
 
Section E Part II: The steps for determining the presence of hallucinations and/or delusions in this section include: 
  • Review of the medical record.
  • Interview of staff. 
  • Observation of the resident during the 5 structured interviews required for the MDS assessment. 
The team is determining whether the patient has made statements which indicate false beliefs. The manual instructs to investigate whether the resident will accept correct information and true facts.  While avoiding an argument with the resident, assessors will determine if the false beliefs are fixed. A key point that the manual makes is, if a belief cannot be proven by the staff to be false, do not code as a delusion.  Coding in this section is based on behaviors observed and/or thoughts expressed in the last 7 days rather than the presence of a medical diagnosis.
 
Rejection of Care: The MDS 3.0 RAI Manual guides assessors to review the care plan when determing if a patient has refused care. The manual states: Do not include behaviors that have already been addressed (e.g., by discussion or care planning with the resident or family) and/or determined to be consistent with the resident's values, preferences, or goals.
