Healthcare Reform Affects SNF Setting

The RUG-IV classification system will not start until Oct. 1, 2011, under the new healthcare reform bill.

The House of Representatives Sunday voted to approve a healthcare reform package that offers a lot of provisions that affect long-term care. Besides delaying implementation of RUG-IV, the bill will retain the Medicare market basket and extend the Medicare Part B therapy caps exceptions process through Dec. 31, 2010.

 
MDS 3.0 Section D: Mood Part II 
 
Upon completion of the 9-item Patient Healthcare Questionnaire calculate the Total Severity Score.  Obtain the Total Severity Score by adding the frequency responses in column 2.  The total score must be between 0 and 27.  The score is useful for knowing when to request additional assessment by providers or mental health specialists for underlying depression.

 

Total Severity Score can be interpreted as follows:

 

1-4:         Minimal depression

5-9:         Mild depression

10-14:     Moderate depression

15-19:     Moderately severe depression

20-27:     Severe depression

 
It is important to note that coding the presence of indicators in Section D does not automatically mean that the resident has a diagnosis of depression or other mood disorder.  Assessors do not make or assign a diagnosis in Section D, they simply record the presence or absence of specific clinical mood indicators. Facility staff should recognize these indicators and consider them when developing the resident's individualized care plan.
