MDS 3.0 Section D - Mood: This section contains the 9-item Patient Healthcare Questionnaire (PHQ-9). The questionnaire is designed to identify signs of depression. It is standardized and copyrighted which results in a severity score and a rating for each patient. The look-back period for this item is 14 days. There are 9 interview questions that are reported by symptom presence and symptom frequency.
Obtaining information about mood directly from the resident, sometimes called "hearing the resident's voice," is more reliable and accurate than observation alone for identifying a mood disorder.
The manual instructs to conduct the interview the day before or the day of the ARD. Conducting the actual interview requires preparation and planning to assure the accuracy of the information obtained from the resident.
Tips for conducting the interview include:
* Conduct the interview in a private, quiet and non-distracting environment.
* Offer the resident an interpreter.
* Sit so that the resident can see your face, with lighting that is not too
bright or too dim.
* Confirm that the resident can hear the assessor.
* Utilize a printed form of the interview if necessary with large print.
* A CUE CARD with printed responses can enhance the quality of the
interview.
* Suggested language: "Over the last 2 weeks, have you been bothered by
any of the following problems?"
* Each question must be asked in sequence to assess presence
(column 1) and frequency (column 2) before proceeding to the next
question.
* Resident may respond to questions:
* Verbally
* By pointing to their answers on the cue card, OR
* By writing out their answers
