CMS Posts RAI Manual Chapters 2 and 4 
 
Harmony is pleased to inform our clients and contacts that CMS has completed the MDS 3.0 RAI Manual with the final post of Chapters 2 and 4.  Chapter 2 addresses Assessments for the RAI instrument. This chapter presents the assessment types and instructions for the completion (including timing and scheduling) of the mandated OBRA and Medicare assessments in nursing homes and the mandated Medicare assessments in swing bed hospitals. Chapter 4 instructs on the Care Area Assessment process and Care Planning. The MDS information and the CAA process provide the foundation upon which the care plan is formulated. There are 20 problem-oriented CAAs, each of which includes MDS-based "trigger" conditions that signal the need for additional assessment and review of the triggered care area. Detailed information regarding each care area and the CAA process, including definitions and triggers, appear in Chapter 4 of the manual. Chapter 4 also contains detailed information on care planning development utilizing the RAI and CAA process.
 
In addition to the newly posted chapters, CMS has revised the MDS 3.0 assessment forms. The updated forms have a new reference number for easy identification. The MDS 3.0 item Subset reference number is V1.00.1.  The updated forms are NOT currently posted in Appendix H in the current MDS 3.0 RAI Manual.  To access the updated MDS 3.0 assessment Subsets you must click the Item Subset Link on the CMS MDS 3.0 web page. The link is located below.
