Does Your Nursing Documentation Depict Skilled Wound Assessments?
Part III
This is the third and final installment discussing the assessment and documentation of wounds to support daily skilled care.
Harmony recommends that each facility have clearly written and accessible to all staff a policy regarding wound care and documentation. Examples of skilled notes should be available to any new staff treating skilled patients. Other Key assessment considerations should be included in wound assessments:
- Immune status
- Blood glucose levels
- Hydration
- Nutrition
- Blood albumin and pre-albumin levels
- Oxygen and vascular supply
- Pain
- Cause of wound
Caretaker education regarding wound care policies should convey an understanding that prevention, care (promoting healing) and treatment of skin breakdown should be a facility wide approach involving all caregivers. Evening and night shift caretakers should be aware of prevention and care measures for each patient and taught to document observations and assessments regarding the care plan for skin breakdown.
