Mapping Therapy Minutes: Part II Last week's article discussed the benefits of mapping out therapy minutes for Medicare Part A beneficiaries on a skilled therapy program. Communication between the Rehab Manager and the MDSC is a key factor in successful Rehab Case Management. Another major area for clear concise communication is between the therapy staff and the nursing staff on the units, to assure that therapy appointments are not missed due to conflicts. Harmony provides the following list of additional strategies.

Give patient and staff a daily schedule for their therapy sessions where possible, especially patients who tend to refuse or attend other activities.

Hold daily minute management meetings.

Therapist to know in advance the patients appointments, dialysis sessions, etc.

Educate family members and all nursing staff on the importance of therapy. If nursing staff and ancillary staff don't understand how important therapy is, care and reimbursement may be sabotaged.

Communicate with nursing staff on a daily basis about patients' conditions, progress, schedule status and anything that is hindering patients from receiving therapy and successful discharge from skilled services, regardless of discharge destination.

It is important that the Rehab Department develop clear communication and tracking policies and decide who is responsible for tracking the therapy minutes to ensure minutes are not lost.
