Q & A for May 27, 2008
This question was asked at a recent seminar presented by Harmony Healthcare entitled "Treating the Cognitively Impaired Patient in the SNF."
Q: How do I set goals for a patient who is cognitively impaired to show that skilled therapy is necessary and the expectation of progress is reasonable and necessary?
A: Setting goals which focus on cognition need to be measurable and patient related.
Harmony suggests the following examples for setting measurable cognitive based goals:
Attention: "Patient will attend to ADL tasks for a 30 minute session with rare cues to regain attention."
Sequencing: "Patient will properly sequence bathing and dressing tasks 3 out of 5 trials with supervision only, using visual aide provided by OT"
Memory: "Patient will demonstrate recall of completion of AM self care when prompted by OT, 60 minutes following completion of session."
Setting goals for people with dementia is similar to setting goals for the other patients we see who have diagnosis such as a CVA, MS or Parkinson's disease. With these patients abilities may decline over time, but patients continue to possess reserve of remaining function. Dementia is a chronic, progressive condition; but therapeutic strategies can help tap into a patient's abilities.
