2011 Marching Band Registration Kickoff!
To all Marching Band & Colorguard Parents:
While students are busy practicing in preparation for band camp, parents should begin now to prepare for Walk-in band registration!
There are a number of forms to fill-out, and important information to read. Physician signatures may be required for some students. Don't wait until the last minute to see what is needed!
Beginning today, all forms can be found at the website: link to Band Registration Documents. Bring all completed forms to Walk-in Band Registration (see dates below).
- An Emergency Medical Information Form, Field Trip Authorization Form, and a Code of Conduct Form (all download-able at the website) MUST be turned in at band registration for each child in band.
- Additional forms may/may not apply to you and your student. Please review at the website and complete as necessary.
- Please note: The signature page of the Handbook is to be given directly to Mr. Grantham by each student.
Walk-in Band Registration will be held on the following days at the Band Room. You may come either day, at the time that is convenient for you. Please bring all completed forms. If you plan to pay your FSC (Fair Share Contribution) by check, please bring it with you, payable to AFM.
Saturday, Aug. 13 9:30-10:30 a.m. and 4:30-5:30 p.m.
Monday, Aug. 15 11:30-12:30 p.m. and 8:00 -9:15 p.m.
