Crestwood Country Day Camp & School Greetings!       We hope you and your family have enjoyed the winter break.  The weather seemed to make it feel more like spring.  Before you know it, summer will be here!  Please see below for important camp forms that will need your attention.  Also, below is a link to view our new Crestwood promo video.   Forms on the Website      We want to take this time to let you know that our medical, personal information and prescription authorization forms are now available on our website. Personal information forms(PI)  Due May 1st Medical forms  Due May 15th Prescription authorization form  Due June 15th      For those of you that are enrolled for the summer of 2012, you should be receiving these forms in the mail within the next few weeks.  New Video!      Crestwood filmed a new promo video last summer that featured some of our parents, campers and staff. It is now on our Facebook page for your viewing pleasure.
