When is training "required" and who is required to be trained when entering a Confined Space?

OSHA and WISHA rules state that the employer must
"provide training to each employee involved in permit required confined space activities, so they can acquire understanding, knowledge and the skills necessary to safely perform assigned duties". This training applies to anyone acting as the entrant, the attendant, or the entry supervisor.
