The meaningful use-compliant electronic health record (EHR) has quickly become very adept at capturing and sharing standardized, structured clinical content that can be communicated, stored, and to some extent consumed by other systems. Unfortunately, this strength is also the EHR's greatest limitation. Amid the structured templates and required fields of the EHR, the essential critical knowledge a provider needs to know is often so obscured that the EHR becomes more of an obstacle or annoyance than a truly useful source of clinical information.

Two Steps to Simplify ICD-10 Transition at Your Medical Practice

There is a lot of panic-inducing marketing going on out there like: "ICD-10 is going to clog up the process so badly that no one will be paid"; "ICD-10 will not be accepted by everyone, which means double submissions in order to be paid by someone who has a secondary policy, etc."

The CCD and Meaningful Use: A Mutually Beneficial Relationship 

As the continuity-of-care document undergoes refinement, some view it as a lynchpin in the quest to effectively share patient information.

The Office of the National Coordinator for Health Information Technology (ONC) opted against specifying standards in its criteria for stage 1 meaningful use. However, in setting the expectations for how information follows patients as they transition across care settings, the agency may inadvertently have given favor to the continuity-of-care document (CCD) over the continuity-of-care record (CCR).

A new AMA report on patient safety in ambulatory care finds that safety problems are widespread but that little is known about which problems cause the most harm. Among the uncertainties cited by the AMA researchers is the contribution of electronic health records (EHRs) to patient safety.READ MORE
Small medical practices greatly at risk for data breaches

Data breach experts are issuing a warning to small practices -- don't be the vulnerable target that data thieves assume you are.

The Top Cyber Security Trends for 2012, as compiled by Kroll's Cyber Security and Information Assurance, reported that small practices are more susceptible to security vulnerabilities because they are "the path of least resistance." Many rely on outdated technology. Basic security protections, such as proper use of encryption, often are overlooked as practices focus on meeting regulatory requirements, such as those related to meaningful use.
