HPV - The Elephant in the Room

April is the official oral cancer awareness month so let's speak openly and honestly.  I need to talk about something uncomfortable.  Let's talk about awareness of HPV (Human Papilloma Virus), a sexually transmitted disease.

According to the Centers for Disease Control and Prevention about 20 million Americans are currently infected and 6 million more will be infected this year alone. 

More than 40 types of HPV are spread during genital, oral and anal sex with an infected partner.  In most cases, the body's own immune system gets rid of HPV.  But if the body cannot clear the infection, several cancers may develop like anal, cervical and oral cancers.

In fact, oral cancers related to HPV increased by 225% between 1988 and 2004, with men accounting for the majority of cases.  A recent report from the Journal of the AMA showed that HPV "peaks" between age 30 - 35 and again at age 60 - 64.
 
Fortunately, oral cancer patients who test positive for HPV are typically younger and fitter, less likely to smoke or drink, and are found to do better than patients whose head and neck cancers are linked to alcohol and tobacco.
 
Dr. Maura Gellison, the lead author of the JAMA study said "the next step in clinical trials will be to determine the effectiveness of the HPV vaccine in preventing HPV positive oral squamous cell carcinoma.  We understand the Achilles heel of the cancer.  If you don't get HPV infection, you don't get the cancer."

Men and women need to bring this up with their doctor.  Your life may depend on it.

Head & Neck Cancer - 

Risks, Signs & Symptoms 

 
Risk Factors - 
The most important risk factors for head and neck cancers, besides what Dr. Hughes just discussed, are alcohol and tobacco use (including smokeless tobacco known as chewing tobacco, dip, chew and snuff).
 
At least 75% of head and neck cancers are caused by alcohol and tobacco use.  People who use both together are at an even greater risk. 
 
Signs & Symptoms -
  • Lump in the neck - Cancers that begin in the head and neck usually spread to lymph nodes in the neck before they spread elsewhere.
  • Change in voice - Most cancers in the larynx (voice box) cause some change in your voice.  If you are hoarse or notice voice changes for more than 2 weeks contact your physician.
  • Growth or Sores in the Mouth - A white or red patch on the gums, tongue or lining of the mouth.  
  • Unexplained bleeding in the mouth. 
  • Swelling of the jaw that causes discomfort or dentures to fit poorly.
  • Swallowing problems - If you notice difficulty swallowing and it is happening frequently you should see your physician.  Cancer of the throat or esophagus may make swallowing solids difficult.
  • Persistent earache - Constant pain in or around the ear with or without swallowing or frequent headaches associated with earache.
  • Persistent pain in the mouth or throat.

All of these signs and symptoms can occur without cancer, but you can't be sure without an examination.  If you experience any of these symptoms you should contact your physician and be evaluated.  Yearly physicals with your primary care physician and routine dental exams that include inspection of your teeth, gums, tongue and throat are very important. You need to speak up and ask for this exam. You are worth the few minutes its take to complete.
