Making medical decisions is harder than ever. Doctors, family, friends, television ads, news anchors, and the Internet - everyone has an opinion.

Even for a medical professional it can be confusing. Each week the medical journals report on conflicting studies. And it is often left to interpretation. In medicine, there is always more than one way to address a problem or treat a cancer. Right now prostate cancer screenings and treatment opinions are a hot topic.

Many physicians rely on evidence-based medicine only. They treat every patient with the same disease in exactly the same way.
But in working with Dr. Hughes and Dr. Marger, I have learned that there is a better way. I call it "Judgment Based Medicine".
 
Dr. Hughes and Dr. Marger take into account the medical facts but really enter into a personal partnership with their patients. Taking into account the whole person to develop the best treatment for that individual patient. Not only do they consider the specifics of the type of cancer, but also other medical problems and life style. It is a shared responsibility.
 
As a patient you want to know the doctor is on your side, helping you choose what is best for you. Be sure that you develop this relationship with all of your health care professionals. Become a real partner in your own healthcare.

Treating Prostate Cancer with CyberKnife

 

CyberKnife is RadioSurgery, not traditional radiation therapy. Prostate cancer can be treated in 5 days instead of 40+ treatments in 9 weeks.
Let me explain why.
 
Targeting accuracy - the ability to hit the cancer and miss normal tissues- is the key to success in any form of radiation treatment. CyberKnife has emerged as the only system capable of non-invasively delivering high doses of radiation with sub-millimeter accuracy - 0.75 mm to be precise. Even better than with IMRT or proton therapy.

 

The prostate can shift 0.5 - 1 centimeter during the course of a single treatment. Therefore without tracking, we treat with a small dose of radiation over many treatments with IMRT and proton therapy.

 

Safe delivery of radiosurgery means continuous tracking and correction. CyberKnife is the only system that uses continual image guidance to track, detect and correct for prostate movement during treatment allowing the necessary radiation dosage to be achieved quickly and accurately.

 

The Stanford University 5 year study showed a 93% cure rate with CyberKnife treatment. Georgetown University showed a cure rate of 95%. These studies also found low levels of bowel and bladder side effects.
 
What does this mean for the man with prostate cancer? It means he can be treated with the CyberKnife in just 1 week as opposed to 8-9 weeks of treatment with IMRT or proton therapy. 

 

I have been doing Stereotactic RadioSurgery for brain cancers since 1988. I look forward to using the CyberKnife's improved technology to treat other diseases like prostate cancer.

Prostate Cancer Screening 

 

The controversy continues. Who? What tests? When? How often? All are debatable. But the fact is that this year there will be

240,890 American men newly diagnosed with prostate cancer and 33,720 deaths as a result of the disease. These numbers are not debatable. (Statistics from the National Cancer Institute)

 

Our physician recommendations are:

        MEN at RISK
             (Family history, African American, or a
              BMI > 25 kg/m - 5'9" man at >175 lbs) 

     At age 45 need:
         * PSA blood test annually
         * Digital Rectal Exam (DRE) annually
 
ALL MEN
     (Regardless of risk)
  
      At age 50 need:
          * PSA blood test (every 2 years minimum)
          * Digital Rectal Exam (every 2 years minimum)

SYMPTOMATIC MEN*
      (Decreased urinary flow, increased urinary frequency, waking up multiple times to urinate, blood in urine or ejaculate)

         At any age need immediately:
             * PSA blood test
             * Digital Rectal Exam
             * Ultrasound of prostate

Prostate cancer screening is still a personal decision between a man and his doctor. Your physician should use "Judgement Based Medicine" to determine your care path.

* Most prostate cancer is asymptomatic, so men do not realize that they have a problem until the cancer has spread.

Christmas Came Early

 

On December 13 at 5 am, the trucks rolled in to deliver the CyberKnife Treatment Machine. It took a moving van, a medium truck, a fork lift, and about 15 men to unload and unpack. It will take 2 weeks to assemble and another 4 weeks to make it work perfectly.
