This note is a reminder that you MUST have "Wet Signature" Player Registration forms with you at all times for practices and games.
A wet signature form is an original form that has been signed by the parent/guardian. Wet signatures are required by emergency personnel should any treatment be necessary, should the parent/guardian not be available.
If your player signed up on-line and all you have is the computer generated sign-up form, then you MUST get the parent/guardian to complete a Player Registration Form too.
For some parents, you may want to complete the form and simply present it to the parent for signature.
Please make sure you have all the player forms with you whenever you are doing anything with your players; practices, games, team events. Hopefully they are never needed BUT in the event they are you don't want to be scrambling. Click on any of the links to get a Player Registration Form.
These forms are also required for any player going on to All Stars at the end of the season.
