The authors say that "diverse groups of residents [may have] varied and sometimes complex everyday needs, which can quickly amplify during emergency or disaster events."  The online toolkit offers information and resources that will help you, your organization and the people you serve connect and communicate.

PSPC is recruiting teams for its fourth year (PSPC 4.0) in which hundreds of communities and organizations will work together in a disciplined and focused model to enhance the ability of a community healthcare safety net to deliver safe care that produces optimal health outcomes for high-risk patient groups. This model is patient-centered, driven by inter-professional teams and supported by systems of partnerships.

To learn more about PSPC and how to participate, join one of the informational calls on Sept. 15 or 20. Register for these calls here.

The built environment- what can you do?

The authors of this month's ACU column argue that problems in the built environment contribute to physical and mental health disparities in underserved communities. What can you do about air pollution, lack of affordable housing, poor transportation, problems with walkability, inadequate food access and a dearth of parks and green designs be done? Read case studies describing successful interventions led by clinicians in The Role of Clinicians to Empower Communities through Utilization of the Built Environment, by Drs. Sarah Kureshi and Kim Bullock, published in the Journal of Health Care for the Poor and Underserved.
