In this third issue of Pacific Hospital Preservation and Development Authority (PHPDA) Update, we report on the community interviews conducted earlier in 2010, the promising results from the Patient Referral Pilot in partnership with HealthPoint Community Health Centers and the Patient Navigator Pilot in partnership with Seattle Children's.

Since the spring, the Pacific Hospital Preservation and Development Authority (Pacific Hospital PDA) Governing Council and executive leadership have conducted a series of interviews with dozens of community thought leaders. We wanted to determine their ideas on how to improve access for the uninsured, the areas of unmet need and the best ways to leverage our limited grant dollars. Our Governing Council reviewed the findings of these interviews and recommended the following priorities for the Pacific Hospital PDA support in 2011.

  • Retain our focus on specialty care access for uninsured and underinsured patients.
     
  • Continue our match for Medicaid Interpreter Services through our interlocal agreement with the Washington State Department of Social and Health Services.
     
  • Support best (or emerging) practices in health care access for underserved or uninsured patients by investing with partners, such as the Patient Navigator pilot with Seattle Children's Hospital.
     
  • Continue to support system change research to eliminate barriers to care.

HealthPoint Federal Way reports initial progress


The Pacific Hospital PDA has been working in partnership with HealthPoint Federal Way Community Health Center

to increase the percentage of safety net patients who are appropriately referred to specialty care and are then seen by a specialty provider. In addition, the clinics wanted to increase the percentage of specialty providers who returned a consult note for the patients Electronic Medical Record (EMR).

 

HealthPoint Federal Way clinic served as the pilot site to delve into the underlying reasons and systems issues that were barriers to meeting these goals. The PHPDA retained MCPP Healthcare Consulting to work with the clinic.In collaboration with HealthPoint Medical Director Judy Featherstone and the Federal Way clinic, PHPDA consultants identified some underlying problems in the external and internal specialty referral processes and developed solutions. The team also identified opportunities for improvements to the NextGen specialty referral template of the automated medical record used by HealthPoint and many safety net clinics.

navigatorPatient Navigator Pilot: One-Year Review

Spanish and Somali-speaking patients served

Hispanic familyThe Pacific Hospital PDA funded a two-year grant to support a patient navigator pilot at Seattle Children's. The first year was completed on June 30, 2010. The grant supports three patient navigators to serve Spanish- and Somali-speaking Limited English Proficient families seen at Seattle Children's for complex medical care or chronic diseases. Clegg & Associates was retained in September 2009 for an independent evaluation of the program to help determine the value and the possibilities for replicating the program in other organizations.

Patient navigators work with families to help them understand their diagnosis and treatment plans, coordinate medical services and assist patients in obtaining financial coverage for their care. They provide interpretation and other services during medical appointments and discharge. They also assist medical staff in understanding the cultural issues families bring to interactions about their child's condition and care.

The Pacific Hospital Preservation & Development Authority (Pacific Hospital PDA) partners with community organizations to improve access to health care services in Seattle/King County.


In 2009, the Pacific Hospital PDA provided almost $2.1 million to enhance access to specialty care and culturally appropriate health services. We anticipate providing $2.1 million again in 2010. In 2011 the Pacific Hospital PDA will celebrate its 30th Anniversary of providing these services to the community.

In 1981, the City of Seattle chartered the Pacific Hospital PDA, placing in its trust the Pacific Hospital and its surrounding campus, a historic Seattle Landmark. The Pacific Hospital PDA leases out the property. The lease revenues are used to support health care access and culturally appropriate care in King County.
