Pacific Hospital PDA Announces:
$125,000 Grant for Patient Navigator Pilot Program at Seattle Children's
SEATTLE, August 31, 2010 -- The Pacific Hospital Preservation & Development Authority (PHPDA) has announced a $125,000 grant for a Patient Navigator Pilot Program at Seattle Children's.
The grant supports three patient navigators to serve Spanish- and
Somali-speaking Limited English Proficient families seen at Seattle Children's
for complex medical care or chronic diseases, such as diabetes. This is the
second year the PHPDA has supported the pilot. "The PHPDA's contribution to the Patient Navigator program directly correlates
with our mission to support culturally appropriate health care and improve
access to health care in our community," said Rosemary Aragon, PHPDA
Executive Director. "In our complex and high-cost health
care system, patient navigators are vital to helping families communicate about
health care over a language and culture barrier."
Patient navigators work with immigrant families to help
them identify barriers to health care and connect them to the resources they
need to make medical decisions around treatment and recovery. Services provided by the Seattle Children's patient navigators include
ensuring families understand their diagnosis and treatment, coordinating
medical services, assisting patients in obtaining financial coverage for their
care and facilitating transportation and child care for medical appointments. They
also provide services and interpretation during discharge and follow up to ensure
that families' needs are met and that they are implementing the discharge plan and ongoing
care as intended.
