Many older adults and adults with disabilities that are leaving the hospital, rehabilitation facility or nursing home are in need of transitional resources.
We know discharge planners, nursing home social workers and home-care professionals understand and use resources. We want to compliment existing roles and support professionals as they assist consumers moving between services and facilities.

Consider consulting with an ADRC Information & Assistance Specialist regarding long term care options counseling and information about community resources.
Some of these resources include: adaptive equipment (when insurance coverage is not available), assisted living/nursing homes, in-home personal care, homebound meals, prescription drug coverage, respite care, support groups, cost of services, eligibility for Long Term Care programs, transportation and falls prevention.
In addition to exposing your clients to the multitude of available resources and support, the ADRC offers Options Counseling - - on the phone and in the home. This is a person-centered, decision support service that empowers older adults, adults with disabilities and their families/caregivers to make informed decisions about current and future long-term care services.
All of us are being challenged by the Federal Government to improve post-treatment success at home for the persons we serve. We all know what a challenge this can be when persons minimize their needs and over estimate their informal supports.
