Meals For The Home Bound
Eligible home bound persons can receive a hot, nutritious meal each week day delivered to their home. You can get this process started by helping clients complete the ADRC Home Bound Meal Application, which is available on the ADRC website. Eligibility requirements are:
- 60 years of age or older, spouse of a person age 60 or older.
- Who are homebound--not currently driving.
- Who are unable to prepare meals for themselves.
- Who have no one to prepare a meal for them and do not reside in a complex that offers meals.
Adults under age 60 and legally disabled may qualify, please call for more information. Meals are delivered Monday - Friday around the noon hour by volunteers.
Professionals can refer clients for this service. Open and print the link below. Assist your client in filling it out and fax to
920-448-5302. A home visit from ADRC staff is required without a professional referral. Applications are subject to eligibility criteria review and route availability. If you are unsure about eligibility, please call for an individual consultation.
