Summer travel plans?
Good for you, but be prepared!
If you are lucky enough to be getting out of town this Summer, you may want to consider preparing yourself and loved-ones for a possible emergency.
For Parents of Children Under 18:
If you go out of town without your children (or send your children off in the care of another), will the person you leave them in the care of have the ability to make health care decisions or even travel with them? Parents should consider preparing a "Designation of Agent to Make Health Care Decisions and Permission to Travel for Children". This document allows you to appoint one or more individuals to make health care decisions for your children and allows such individuals to travel with your children, which would include taking them to and from school and activities. Please contact my office if you would like this document prepared.
For All Clients:
If you are traveling out of town, it is a good idea to bring along copies of some of your documents, including your Medical Power of Attorney, HIPAA Release Authority, and Statutory Durable Power of Attorney for financial needs. In lieu of carrying hard copies or originals, I have provided all of my clients with electronic PDF copies of their documents on a CD. If you have misplaced your CD or would like another, please contact my office. You may also want to save the electronic copies onto a flash drive for easy travel.
