The Revolving Door "One quarter of all US hospital patients are readmitted over a two-year period for the same conditions that led to their original hospitalizations.""Among Medicare patients, 42 percent had multiple hospital admissions and 38 percent had multiple emergency department visits.""Among Medicaid patients, 23 percent experienced multiple hospital admissions and 50 percent had multiple emergency department visits." (Health Day 6/3/2010) These numbers are driving the effort to reduce re-admissions. They represent enormous cost, much of it preventable. We believe the solution is better outpatient monitoring and in-home care. Hospitals and emergency rooms are designed for treatment of acute illness. The rapidly growing senior population has chronic conditions that need gentle management. A visit by a caring person who knows the client is the best way to provide that. Countries that have successfully managed an aging population ....
