Sometimes, the best way to find out about your failings is to send incorrect information to 200 people in an email. You are bound to find someone who can point out your error. Thanks to those who know more than I and have led me in the paths of righteousness. Please note the change to passport information:

You can pretty much ignore what I wrote before in this section except for notarized letter of consent. That was and is accurate. The problem is the actual passport. I was correct in stating that you did not need a passport to enter Canada. But if you want your child to be allowed back into the good ole U.S. of A.....? Every person on tour needs to have a passport. A passport card is cheaper than the book and is acceptable for Canada and Mexico. If you are going, say, on mission trip? Get the book if you might be going to any country other than Canada and Mexico in the foreseeable future.
I was going to try to have all necessary information included in this email, but there is too much. Your best bet is to go to this website and figure it out for yourself. There are different requirements for first time applicants, persons under 16, etc. Make sure you follow all necessary procedures. I would suggest doing it over Thanksgiving break, otherwise you might have to pay the expedite fees. For students under 16 years of age, both parents (guardians) must be physically present during the application process.
Some suggestions to help make the process work smoothly:- Birth certificate MUST be an "Official" County Form (with raised seal, etc.). No mini/souvenir certificates.
- Be prepared to pay for the passport fees by cash, check, or money order.
- Either get photo taken in advance or be prepared to pay for that with cash, check, or money order as well.
And after all the hassel and rigmarole of passports, you still need to have notarized letters of consent. Sorry.
- Notarized letter of consent. If there is a joint custody situation, you must submit multiple notarized consent letters (one for each parent/guardian), or else have all parents/guardians with legal custody included on the single consent letter. Download the letter here or on the sidebar.
