Ask the Expert! The Patient With Advanced Dementia |
The previous articles in this series have focused on the patient with new onset dementia. This article reflects a change in focus. It is based on a recent article published this past month in the New England Journal of Medicine.[1] The article deals with the reality of patients with advanced dementia living in a nursing home. The authors tracked the clinical course of 323 dementia patients living in 22 nursing homes in the greater Boston area. The conclusions are quoted: "Pneumonia, febrile episodes, and eating problems are frequent complications in patients with advanced dementia, and these complications are associated with high 6-month mortality rates.... Patients with health care proxies who have an understanding of the prognosis and clinical course are likely to receive less aggressive care near the end of life."
