To determine whether the increased prevalence of
low
serum cobalamin concentrations in elderly people
represents true deficiency, serum concentrations of
cobalamin and folate and of metabolites that are
sensitive indicators of cobalamin deficiency were
measured in 548 surviving members of the original
Framingham Study cohort. Serum cobalamin
concentrations < 258 pmol/L were found in 222
subjects (40.5%) compared with 17.9% of younger
control subjects (P < 0.001). Serum methylmalonic
acid and total homocysteine concentrations were
markedly elevated in association with cobalamin
values < 258 pmol/L in 11.3% and 5.7%, respectively,
of the cohort. Both metabolites were increased in
3.8% of the cohort, associated with significantly lower
erythrocyte counts and higher mean cell volumes.
Serum metabolites correlated best with serum
cobalamin values, even when subnormal
determinations were excluded. The prevalence of
cobalamin deficiency was > or = 12% in a large
sample of free-living elderly Americans. Many elderly
people with "normal" serum vitamin concentrations
are metabolically deficient in cobalamin or folate.

Neusspray, een verrassend perspectief
In zijn promotieonderzoek "Nasale toediening van
migrainemedicijnen' beschrijft Hugo van der Kuy zijn
bevindingen bij de effecten van "vitamine B12 dat
direct in de bloedbaan wordt opgenomen via het
neusslijmvlies". Want zijn methode: neusspray, bleek
verrassend perspectief te bieden.
Vitaminespray werkt bij migraine
Een neusspray met vitamine B12 helpt tegen een
migraineaanval, zo heeft promovendus Paul-Hugo
van der Kuy ontdekt.
Hij constateerde dat tien van negentien onderzochte
migrainepatiënten baat hadden bij de neusspray. Zij
kregen tenminste 50 procent minder aanvallen, hun
aanvallen duurder korter en ze hadden minder
medicatie nodig.
Vitamine B12 (oftewel hydroxobalamine) is een
mogelijk medicijn tegen migraine omdat de vitamine
stikstofoxide wegvangt, het molecuul waarmee een
aanval waarschijnlijk begint. De vitamine wordt via de
mond echter slecht opgenomen. Bovendien hebben
misselijke migrainepatiënten moeite met het slikken
van pillen en vervallen tijdens een aanval de normale
maagbewegingen. Een neusspray heeft die nadelen
niet.
Alhoewel de grote daling van het aantal aanvallen
opmerkelijk is, vergeleek Van der Kuy zijn neusspray
niet met een placebo. Ook blijkt de spray niet te
werken voor vrouwen met menstruatie-afhankelijke
migraine.

Efficacy of a Pelargonium Sidoides Preparation in
Patients With the Common Cold: A Randomized,
Double Blind, Placebo-Controlled Clinical Trial.

BACKGROUND: The common cold is a viral infection
with symptoms such as sneezing, sore throat, and
running nose. It is one of the most prevalent illnesses
in the world, and although commonly caused by
rhinoviruses, antibiotics are often prescribed
unnecessarily. Therefore, it is of utmost importance to
evaluate alternative treatments such as herbal
medications, whose efficacy and safety is proven by
pharmacological and clinical studies.
CONCLUSIONS: EPs represents an effective
treatment of the common cold. It significantly
reduces
the severity of symptoms and shortens the duration of
the common cold compared with placebo. The herbal
drug is well tolerated.

For centuries the roots of Pelargonium sidoides DC
have been used in South African ethno-medicine for
the treatment of respiratory diseases. In Germany, a
medicinal product containing a special extract of this
substance is among the group of self-medication
products most widely bought at chemist's shops. In
December 2005, the Federal Institute for Drugs and
Medical Devices (BfArM, Bonn) approved a new
license for its use as a drug. The following review
focuses on the current pharmacological, toxicological
and clinical data covering the efficacy and
innocuousness of this drug when administered for
the treatment of acute bronchitis.
