Meet Erin Vogt, Out New Care Advocate |
"I became a geriatric social worker
because of my amazing relationship with my grandparents. Because of them, I have always seen older adults as wonderful people. Even as a kid, I wanted to own a Laundromat and a nursing home. Working with older adults and the people who love them allows me to be me, and, in some small way, honor my grandparents."
Erin C. Vogt, MA, LCSW, ACSW holds a Master's Degree in Social Work from Aurora University, as well as a Master's Degree in Organizational Leadership from Lewis University. Her passion for the older adult population led to a specialty certification in geriatric social work from the National Association of Social Workers. Most recently, Erin served as the Manager of the Alexian Brothers Hospital Network's Resource Center for Seniors and coordinated the satellite program of Chicago Lighthouse for the Blind and People Who Have Low Vision.
Erin is also a Certified Care Manager and is an Adjunct Assistant Professor at Lewis University in the Department of Justice, Law, and Public Safety Studies and at Aurora University in the School of Social Work. Erin and her husband, Peter, have two children, five year old Johnathan and three year old Hailey.

| What is Life Care Planning and What Does a Care Advocate Do? |
 |
Life Care Planning is a relationship and a planning process involving the client, the client's family or support system, and the professionals at Janna Dutton & Associates. This relationship places special emphasis on the issues that develop as you go through the later stages of your life, and it provides knowledge and expertise in the face of your questions and concerns. The purpose of the Life Care Plan is to establish quality care, help with decisions regarding long term care needs, and find sources to finance quality long term care.
The Care Advocate's role is to provide information and answer questions needed for the client and family to understand and make decisions about the client's particular health care needs. Care planning is made up of a lot of moving parts. The goal of Care Advocacy is to work with you and for you to help fit all these pieces together. This means helping clients by (but certainly not limited to):
· Working to achieve holistic care, including the physical, emotional, mental and spiritual well-being of the client
· Guiding the client and client's family to anticipate and plan for inevitable changes, as well as react to sudden ones
· Assisting in understanding public benefits that may help pay for needed care

Life Care Planning on Our Blog |
Late last year, The New York Times published an article on the benefits of care advocates, also referred to as geriatric care managers. Our blog on the article discussed the common temptation for people to handle every aspect of caregiving themselves. It can be difficult to allow someone else into this kind of discussion, but sometimes outside help can actually allow you to provide the best possible care for your loved one, not to mention allow you to take better care of yourself. These advocates have both the time and the expertise to plot out a plan that family members often do not, which can greatly improve your elder's quality of life.
