Blue Cross Medical Insurance Packet - Important Title
Those members who are on the SCDA group medical plan and own a practice of less than 20 employees should have received a packet of information from Blue Cross recently pertaining to the state continuation subsidy plan. As you may recall from previous e-grams and newsletters, the administration in Washington signed a plan into law February 17, 2009, that provides a premium subsidy for those who have involuntarily lost their job between the dates of September of 2008 through December of 2009. If your practice has not had to involuntarily terminate any employee on the medical plan during this period of time, the only thing you need to do at this point in time is sign the one page amendment contained in the above package you received and either mail it or fax it to Rebecca Finley at Blue Cross. If your practice has involuntarily terminated an employee on the medical plan during the above period of time for any reason other than gross misconduct, you will need to follow the steps contained in the packet in order to offer this subsidy. In addition to the one page Amendment, you will need to complete the State Continuation Premium Assistance form as well. Also enclosed in this packet is a 3 pager and an application form that would need to be completed by the terminated employee and mailed back to you, where in which you would return those completed forms to Rebecca Finley at Blue Cross.
