The Journal is launching a new department - Readers Write: Insights From Practicing Correctional Health Professionals - featuring readers' mini-essays on timely topics. To focus the exchange, each issue of the Journal will address one topic.
 
We are currently seeking comments on this question: When is a decision not to treat the right choice?
 
Essays should be 300-500 words, double-spaced and include your name, credentials, job title, professional affiliation, city/state. Also include a phone number where you can be reached. NCCHC's Clinical Guidelines: This FREE Resource Just Got Better!
NCCHC's clinical guidelines are invaluable for correctional health professionals who assess and manage chronic conditions. The addition of three forms for use in daily practice gives clinicians the tools they need to implement a complete monitoring program.
 
The three new forms simplify and  standardize collection of baseline medical data for new chronic care patients, chronic disease clinic follow-up data and clinical data obtained during nursing visits between clinic visits.
 
The guidelines and forms, along with two companion documents-definitions of disease control and clinical status and instructions for using the forms-are available for free at the NCCHC Web site, www.ncchc.org.
