 I found this recipe in a Prevention magazine and it is uncomplicated and delicious. Instead of using the predictable canned fish this recipe uses fish fillets that are quickly poached. This makes the cakes light and the citrus adds freshness. Give these a try on Day 3 of the 5DPT or any time you want a light protein-dense meal. Ingredients: 1 lb cod*, cut into large chunks 1/2 teaspoon salt 1/4 cup + 2 tablespoons 2% Greek yogurt 1/4 cup chopped fresh parsley 1 egg yolk, beaten 1 tablespoon Dijon-style mustard 1 tablespoon freshly squeezed lemon juice (from 1/2 lemon) 3/4 cup + 2 tablespoons or bread crumbs 3 scallions, minced 1/4 teaspoon freshly ground black pepper 1/4 cup canola oil
Directions: STEAM fish: Put about an inch of water in bottom of large nonstick skillet and bring to a simmer over medium-high heat. Season fish with 1/4 teaspoon of the salt and add it to the pan. Cover pan and simmer fish over low heat until just done, 6 to 8 minutes. Remove from pan with slotted spoon and drain on paper towels. Pour out the water and dry pan. Allow fish to cool slightly, about 5 minutes, and pat completely dry. FLAKE fish in medium bowl with forks or your fingers, removing any bones as you go. Add yogurt, parsley, egg yolk, mustard, lemon juice, 6 tablespoons of the panko, scallions, pepper, and remaining 1/4 teaspoon salt. Stir to combine. SHAPE mixture into eight round cakes. Coat cakes with remaining 1/2 cup panko and pat off the excess. HEAT 2 tablespoons of the oil in the nonstick skillet over medium heat. Add cakes and cook till brown and crisp, 2 to 3 minutes. Add the remaining 2 tablespoons oil, turn cakes, and cook until golden brown on the other side, 2 to 3 minutes longer. drain on paper towels. Serve hot with one of the dipping sauces. *I used thawed tilapia fillets and they worked quite well. Select a firm white fish that is free of bones and skins. Be sure to safely thaw frozen fish fillets following package instructions. Serves 4. Per serving: 291 calories, 29g protein, 12g carbohydrate, 16.5g fat (2g saturated), 1g dietary fiber. 5DPT Recipes Day 3 |

But I Gained Some Weight Back Anyway
"Dear Kaye,
Thank you for the Day 6 book. I guess I am one of "those people" - had the surgery and lost and then it came back, the weight. My WLS nurse warned me I could regain the weight and I didn't take her serious or maybe I thought I was more determined than everybody else or smarter or something. I wish I had asked her what would cause me to gain the weight back. I'm not sure that back in 2007 I ever heard of slider foods or liquid rules or anything like that. I just knew WLS was the answer for me. So now I'm up 48 pounds and want to stop it before it gets to 50 pounds and all of the sudden another 50 and I'll be back where I started. Thanks for explaining so many things in the Day 6 book. I wish I read this book before I even had the surgery, maybe I wouldn't be one of "those people" or at least not a 48-pound-regain one of those people.
This is the first time I have hope since the re-gain started.

I am not asking for "followers" or discrediting those with different points of view. Like anyone who feels they have been occasionally misrepresented, I just want my point of view to be heard. I always give tremendous credit to our weight loss surgery Neighbors because you are tireless researchers in pursuit of increasing the knowledge you already have to build and sustain a healthier life. So add this to your body of knowledge and draw your own conclusions. I suppose if there is one message above any other I wish for you to hear it is that I truly deeply care for the health of every person in our weight loss surgery community. My life's work is dedicated to serving you and empowering you. As people in the battle against obesity we have taken enough "tough love" and unsolicited unkind advice to last a long lifetime. Answering the 5 Day Pouch Test Critics

As you can see, she struggles with her own weight management issues - in part due to my soft spot for giving her (too many) treats. It is a strange twist of fate that my work is to empower others in their fight against morbid obesity, yet here I am an enabler of my sweet KeepHer and her bulging belly. I suppose in this journey we call life the mission is to try a little harder, to learn a lot, be kind to one another, and just do the best we can. So here are some common criticisms of the 5DPT and my answers. I know this program isn't needed by every WLS patient and I know there are other ways to get back on track. I also know that thousands of people have successfully used the 5DPT and feel empowered by what they learn in the process. So if the 5DPT happens to help you to try a little harder, learn about yourself, be kind to yourself and do the best you can then I have done my job and you have done yours. Best Wishes to you, Kaye & KeepHerKitty "I know people who have done it, lost weight and as soon as it was over just gained it back. It doesn't work."The motivation for doing the 5DPT should always be to get back on track with the WLS dietary guidelines, not to lose weight. That means we take what we learn during the 5 days and apply it to our lifestyle on Day 6 and beyond. If we do the 5DPT simply to knock-off a few pounds and then go back to the very habits that lead to weight regain we will, naturally, regain the weight and then some. When we consented to weight loss surgery we agreed that for the rest of our life we would follow certain dietary guidelines. If we have drifted from the original guidelines the 5DPT can get us back to basics. At that point we must follow the guidelines we agreed to if we wish to sustain weight loss and keep our obesity in remission. "Use this as a means to return to following the instructions you were provided by your surgical weight loss center and nutritionist. Go back to doing what worked for you when you were at your best and losing weight or maintaining weight." (page 24 - 5 Day Pouch Test Owner's Manual). Learn About: WLS Split-Personality Syndrome"The 5DPT is dangerous to my health."This criticism is vague and confusing to me. I think the concern is over the two days of liquid meals or the perception that the 5DPT is calorie restrictive. Consuming a diet of liquids for two days is often prescribed in the recovery and healing from digestive illness or digestive distress. When used correctly the 5DPT heals our digestive system by pampering it with easily digested liquids for two days followed by a methodical three day progression to the diet we were prescribed as weight loss surgery patients. The logical dietary progression of liquids to soft proteins to firm proteins allows a person who has been in a cycle of eating processed carbs to break the habit without a great deal of discomfort. People who are in a serious carb habit and quit carbs "cold turkey" going back to a high protein diet without a progression rarely make it past noon the first day. The plan is designed to gently help your body transition from an unhealthy eating pattern back to the diet prescribed with WLS. I believe a prolonged diet of soft processed carbs is far more dangerous to our health. Read Days 1&2 of the 5 Day Pouch Test Some people attempt to repeat Days 1&2 of liquids for several days in a row. This is not recommended and anything beyond two consecutive days of liquids is not the 5 Day Pouch Test: it is a liquid diet. A liquid diet is not advised unless under the supervision of a qualified health care professional. "Unless you do everything perfectly it is a waste of time."I have never done the 5DPT perfectly and I do not know anyone who has. Rather than make the 5 days about perfection make this time about learning. Little mistakes will not stop the world from turning. So forge ahead doing your very best and forgive little mistakes. Learn from each day, make notes on your 5 Day Pouch Test Journal, and emerge on Day 6 knowing more about yourself and how to work your tool. You can do this!
If we complete the 5 Day Pouch Test having learned a few things about our self, our weight loss surgery and our capability in managing life and weight loss surgery in relationship to one another we have succeeded: we can deem the 5 Day Pouch Test a success. Here are some things to consider while treating the 5DPT as a learning experience: -- What can I eat that gives my pouch a feeling of fullness? What do I eat that fails to give my pouch a feeling of fullness? -- Have the liquid restrictions become automatic to me? Do I have heightened awareness of how I consume liquids with my meals and snacks? Refresher: Lots of Water & Liquid Restrictions-- Am I eating protein in a ratio of 2 bites protein to 1 bite complex carbohydrate? (2B/1B Rhythm) Article: Why 'Protein First' helps us lose weight-- Have I found time to include physical activity in my daily routine? -- Am I allowing myself to feel empowered when I make choices that nourish my body and respect my weight loss surgery? -- Am I forgiving lapses in compliance with my guidelines and moving forward to make better choices the next time? |

Why Soups with Meat & Beans?

How can soups with beans and meat work for a "liquid diet" on Days 1 & 2?
The 5 Day Pouch Test calls for two days of protein rich liquids. Normally we think of ready-to-drink protein beverages or homemade concoctions using fruit, yogurt and protein powders. This is quite typical of the early post-op diet prescribed by many surgical weight loss centers. The 5DPT begins with two days of protein liquids in order to baby the pouch, much as we did immediately post-op. In addition, the liquids are useful in breaking a processed carbohydrate snacking habit or slider food addiction.
Ideally one would spend Days 1 and 2 drinking only protein drinks, clear broth, tea and water. However, depending on your food habits leading into the 5DPT this can be quite drastic causing hunger, dizziness and frustration. As I developed this plan I learned that more substantial soups made of animal protein, legumes, beans and low-glycemic vegetables work well to alleviate the discomfort of a liquid diet.
These satisfying soup recipes are made of foods low on the Glycemic Index: a measure of how your blood glucose levels are affected by food. That means they will stick with you without causing a rapid rise (and then drop) in blood glucose. These great comfort soups will help keep you feeling full longer, help you achieve and maintain a healthy weight, and provide you with more consistent energy throughout the day.
It is easy to confuse soup with slider foods since both are liquids that flow more rapidly through the stoma than solid protein. But the thing to remember is the soup recipes we recommend are nutrient dense. Slider foods such as crackers or pretzels washed down with liquids have no nutritional value.
A final note, the soup recipes we offer are less expensive than processed protein beverages and they are family friendly making the 5 Day Pouch Test more practical to incorporate into our busy schedules.
