Three factsheets and seven slides are now available that highlight some of the latest data on palliative care, advance care planning and Californians' attitudes towards death and dying.
Free POLST Courses for Nursing Homes
CCCC and the USC Davis School of Gerontology invite nursing homes to enroll in a study that will administer and compare three different POLST (Physician Orders for Life-Sustaining Treatment) courses, each designed to teach nursing home providers how to improve use of POLST in their facilities. This study is funded by the Archstone Foundation. The deadline for applications is now July 10. Click below for more information:

A two-day train-the-trainer program for healthcare providers designed to provide an understanding of POLST and how to complete the form to reflect a patient's wishes, as well as ideas for implementing POLST within communities and training other individuals. Join us to learn more about POLST and the art of having the POLST conversation.

California Updates
Citrus Valley Receives Grant from the Lance Armstrong Foundation
Citrus Valley Health Partners was one of 20 hospitals and cancer centers across the U.S. to receive a $10,000 grant toward Joint Commission Certification. In addition to the grant, the Center to Advance Palliative Care will provide tools and training to each of the 20 sites to assist in their efforts to qualify for certification.

The award honors programs that are striving to provide effective, patient-centered, timely, safe, efficient, and equitable palliative and end-of-life care. All U.S. organizations or groups that provide palliative or end-of-life care are eligible for the award.

"Open to Options" Up and Running
Open to Options, a Cancer Support Community program, is designed to help bridge the communications gap between physicians and patients and inspire patient confidence in making treatment decisions. It provides professional counselors who work with patients to formulate a list of personalized questions and concerns to be taken to the oncologist's office. The program is available through the Cancer Support Helpline at 1-888-793-9355 or by visiting their web site.
Palliative Care Best Practices: A Guide for Long-Term Care
The Center for Improving Value in Healthcare's Palliative Care Task Force has updated a step-by- step, comprehensive guide to providing quality care for long-term care residents. It includes advance care planning, pain and symptom management, spiritual and emotional issues and more.
