California POLST Form
Revised POLST Form Effective April 1, 2011
A new POLST form went into effect in California on April 1, 2011. Previous versions of signed POLST forms remain valid. Changes to the California POLST Form were approved by the statewide POLST Task Force and the Emergency Medical Services Authority Commission. For a summary of the changes to the form, click here. Translations are available in Chinese, Farsi, Hmong, Korean, Russian, Spanish, Tagalog, and Vietnamese, as well as Braille (click here to request a Braille POLST form).

POLST Conversation Video Now Available
Facilitating Meaningful Conversations about Goals of Care is an educational video designed for healthcare providers who are responsible for having POLST conversations with patients and families. Eight different scenarios depict realistic clinical situations in which POLST might be introduced, and a demonstration of how the conversation might go. The video was made possible by a grant from the California HealthCare Foundation, and the volunteer efforts of the POLST experts who participated in the video.
The video may be purchased by contacting CCCC. The CCCC member price is $25 and the non-member price is $32.50 (excluding sales tax and shipping). The videos may also be accessed on CCCC's YouTube Channel.

CCCC's popular CPR and Tube Feeding fact sheets have been translated into six languages:

POLST Education
Training in the POLST Conversation
CCCC and the Alameda-Contra Costa POLST Coalition are pleased to present POLST: Beginning the Conversation, a one-day POLST workshop introducing healthcare providers to the California POLST form and how POLST fits into the continuum of advance care planning. Attendees participate in interactive exercises to enhance their skills in facilitating meaningful conversations with patients and their families about goals of care and POLST, and completing the POLST form to reflect patient's treatment wishes. Pre-registration is required and space is limited. Continuing education provided.

National Healthcare Decisions Day
April 16th marks National Healthcare Decisions Day and encourages Californians to think about and plan for the types of treatment they would want if they become seriously ill. That makes it the perfect time to introduce POLST as part of this planning process. Visit the CCCC website to access Advance Care Planning and POLST resources.
Judy Citko Named Vice Chair of the National POLST Paradigm Task Force
CCCC Executive Director, Judy Citko, JD has been appointed Vice Chair of the National POLST Paradigm Task Force. The task force is made of members from endorsed POLST paradigm states who contribute their individual expertise and experience and help to develop national POLST policy and education.
National POLST Paradigm Conference 2012
Save the date for the National POLST Paradigm Conference to be held in San Diego on February 16, 2012. The educational conference will bring together leaders from states that are working on POLST implementation as well as those working on developing POLST programs. California is excited to be the host state for this high-profile event!
