Dear Day Middle School Families,


Massachusetts General Law requires that student's height and weight be measured in grades, 1,4,7 and 10.  The results of the height and weight will be used to calculate Body Mass Index (BMI).  BMI is a screening tool that provides a guideline for determining whether a child is underweight, over weight, at risk of overweight or at a healthy weight.  The purpose of the screening is to identify possible health risks that might prevent students from obtaining the maximum benefit from their educational opportunities.
 
The growth screening will be done privately and with respect for each student.  Students will be asked to remove their shoes. Growth screening results will be kept confidential and sent home by mail.  The information will be recorded on the student's health care record.
 
The F.A Day Middle School will be measuring height and weight of each child in grade 7 the week of February 8th-12th. If you choose not to have your child participate in the Growth Screening Program, I must receive this request in writing.
 
All students in the 7th grade will also have hearing and vision screening.  Scoliosis screening will be done on all students.

I also wanted to pass along a reminder about the free family H1N1 flu clinic scheduled for this Wednesday at Brown Middle School. Details are below.

A family H1N1 flu clinic is scheduled for Wednesday, January 6 from 4 to 6 PM at the Brown Middle School Cafeteria, 125 Meadowbrook Road. All children age 6 months through 18 years old, and their family members are eligible to attend this clinic. Both H1N1 flu shots and the H1N1 nasal spray will be available.  Both first and second doses (for children under 10 years) will be available.

The vaccine is free.


No appointments are needed, but to get vaccinated more quickly, download the two forms you need ahead of time and bring them to the clinic. Vaccine information is also available online with the forms. A set of forms is needed for each person to be vaccinated. All children must be accompanied by their parent/guardian.
