Over 13 million people in the U.S. have early stage to moderate CKD and don't know it. These people will progress to dialysis and kidney transplant without proper medical attention.
Another 13 million know it, but still are at risk of disease progression unless their primary care providers are aware of and attentive to chronic care guidelines. For providers that are part of emerging ACOs, medical home projects, or risk sharing contracts, this is a big challenge, and eventually a costly problem if left unattended.
