November 28, 2011 (Burlington, VT) -- Patient Engagement Systems (PES), a health-care technology company that provides solutions for improving primary care for people with chronic diseases, will unveil a solution for early identification and improved management of Chronic Kidney Disease (CKD).  The solution provides a system for primary care physicians to manage patients within Kidney Dialysis Outcome Quality Initiative (KDOQI) treatment guidelines.  PES will be presenting "Engaging Patients for Treatment of Early Stage Chronic Kidney Disease" on Tuesday, December 6th at the Institute for Healthcare Improvement (IHI) 23rd Annual National Forum on Quality Improvement in Healthcare in Orlando, Florida.


PES technology improves coordination, compliance and the overall management of chronic conditions such as CKD by increasing communications between patients and their healthcare provider and by supporting physician decision making.  With moderate to severe CKD afflicting eight million Americans and with at least as many being undiagnosed, it is critical to remove the barriers to information and treatment in order to take control of kidney health.

 

"It is difficult to recognize early-stage CKD. There are no symptoms until later stages and kidneys begin to fail, yet CKD is manageable if detected early.  In addition, managing CKD to current practice guidelines requires more time than primary care physicians have available," explains Jim Rose, Senior Vice President of Business Development for PES. "PES offers a web-based solution that enables primary care physicians to recognize CKD early and engage the patient into a care plan that is based on KDOQI guidelines."
 

At its IHI conference presentation, PES will present data showing how earlier identification of chronic patients and greater attention and adherence to chronic care guidelines results in dramatic improvements in patient outcomes and quality of life. National Institute of Health (NIH) funded research in diabetes has shown average cost savings per chronic patient of more than $2,400 per year using the PES technology.  "We expect savings in CKD to be at least as much," Rose suggests.


PES technology is the only chronic care management and patient engagement platform that has demonstrated effectiveness in NIH funded studies. The PES program was recently awarded the URAC Best Practices in Health Care Consumer Empowerment and Protection Bronze Award for 2011.
