The Oklahoma City Area Inter-Tribal Health Board Tribal Epidemiology Center (OCAITHB-TEC) would like to request your participation in the Tribal Needs Assessment online survey. The goal of the Tribal Needs Assessment is to gather input from health care directors, providers, and staff to determine how the Tribal Epi Center can most effectively serve the tribes in Oklahoma, Kansas, and Texas. The Tribal Epi Center has developed this survey to prioritize the health concerns in your tribal community. With a priority list of health concerns for your community, the Tribal Epi Center will be able to plan future projects and search for funding opportunities to fit your needs. Your responses to this survey will specifically inform our application for the next round of the TEC grant. The TEC grant RFA will likely be released before the end of 2010 by the Indian Health Service Division of Epidemiology and Disease Prevention.
As the Health Director of your tribe, we ask that you please take the survey and also forward it to all of the staff members that you feel could contribute their knowledge and insight about the health priorities of your community. The survey is only 14 questions long and should take less than 10 minutes to complete.
